In order to participate in GLAYF all athletes must have the following forms on file.
NO PLAYER WILL BE ALLOWED TO START PRACTICE OR CONDITIONING WITH OUT HAVING THESE FORMS ON FILE

Physical Form:

Please have your athletes provider complete the above form. A current physical is one completed by your physician and dated on or after April 15th of the current Football season that your athlete is signing up for.

Concussion form:

Parent waiver release:
parent-waiver-release-form.pdf    -  When you registered your child you should have reviewed this waiver and excepted it by check the box. If you would like to review it or need to print a hard copy please do so.

Concussion return to play form:

If your athlete suffers a concussion, they will need to have this form filled out by their physician to obtain clearance to return to play.

-  All players must participate in each half, and participate in at least 10 live plays during the game.  Exceptions to this rule are allotted for disciplinary reasons only.

Coaches Official Roster Form:

   -  Coaches MUST use the following form to complete a roster and turn it in to GLAYF.  This roster form will be made available to MMPFL.  YOU MUST adhere to the instructions on the form and sort your roster by jersey number in ascending order.  Your players will not be announced unless this form is completed correctly.  If you need instructions, please contact the board or a veteran coach.
